Caren RPM is a SMART® on FHIR remote patient monitoring platform that connects everyone who cares for the patient—caregivers, practitioners, family, home health agencies, pharmacies, and clinics. Caren gives healthcare providers a holistic and historical view of the patient’s health by sharing up to eight measurements and six observations as well as providing a single, updated medication list that can be shared across EHR systems and various levels of care. Caren aggregates health data from multiple sources, such as IoT, wearables, and Bluetooth® medical devices, allowing physicians to gain a full understanding of their patients' health and wellbeing.
